At least 44,000 Americans die each year as a result of medical errors and "near misses" in doctors' offices, hospitals and pharmacies. Luckily, a new law calls for the first national system for tracking medical mistakes. The legislation allows health-care professionals to report errors to a federal database, which researchers will analyze to identify problems. Eventually, the public will have access to reports summarizing the findings on the U.S. Department of Health & Human Services' website (www.os.dhhs.gov). In the meantime, ask your doctor questions and double-check that prescriptions are correct.
